In exchange for participation in the activity of Train the Trainer Course organized by Body Contouring & More, LLC and I; I agree to the following: 

 

1. AGREEMENT TO FOLLOW DIRECTIONS. I agree to observe and obey all rules and warnings and further agree to follow any oral instructions or directions given by Body Contouring & More, LLC during the training course.

2. ASSUMPTION OF THE RISKS AND RELEASE. I recognize that certain inherent risks are associated with body sculpting activities. I assume full responsibility for personal injury to myself, and further release and discharge Body Contouring & More for injury, loss, or damage arising out of my use of machinery. I also acknowledge that BCM is not responsible for my success or future in body sculpting.

3. INDEMNIFICATION. I agree to indemnify and defend Body Contouring & More against all claims, causes of action, damages, judgments, costs, or expenses, including attorney fees and other litigation costs, which may in any way arise from my use of machinery.

4. FEES. I understand and acknowledge that deposits/payments for the above training and/or consultation, purchase of bundle(s), equipment product, and any other purchase are non-refundable.

5. NON-COMPETE. I understand that Body Contouring & More, LLC will be disclosed or may disclose information, including ideas, inventions, and other technical, business, financial, and product development plans and information, during the training/business class. I understand that this is "Proprietary and Confidential Information". I agree to hold BCM Proprietary Information in confidence and to take all necessary precautions to protect such Proprietary Information. I agree not to share or give away any information provided to me from Body Contouring & More, LLC, and understand that legal actions can be taken against me. The agreement shall be governed by the laws of the State of California and may be enforced in any court in California.

6. CONSENT TO USE IMAGE. I hereby give, Body Contouring & More, and Body Contouring & More Academy, consent to photograph and/or videotape my image and likeness.
I also consent for the said image to be used on any Body Contouring & More social media platforms and websites that are, or will be, on the internet publicly.
I understand that no special compensation will be provided for me for any of my image/video usages. In addition, I may not be informed in advance of the specific use of my image.

DISCLAIMER: As of today in the USA, there is no federal certification, national board, or other national licensure requirements to be a body contouring professional and/or to operate and deliver non-invasive body contouring services, since it's not regulated by the Federal Government.

I HAVE READ THIS DOCUMENT AND UNDERSTAND IT. I FURTHER UNDERSTAND THAT BY CHECKING THIS BOX ON THIS RELEASE, I VOLUNTARILY SURRENDER CERTAIN LEGAL RIGHTS.
